Sir, I am holding a CGHS permanent card with entitlement for only in-patient treatment. I do not have the facility for treatment as out-patient as I opted for drawing of the cghs allowance now drawn at Rs.300/-. Now I want to opt for both outpatient and inpatient treatment as I am unable to afford the expenses for outpatient treatment. In this connection, I agree for the stoppage of the cghs allowance of Rs.300 now being drawn by me with immediate effect. I request your goodself to kindly advise me as to how I should proceed for the purpose. An early compliance is requested. PB Menon. Date: 27/04/12.

dear sir my father worked in GREF [part of army] We had a CGHS card at that time but after retirement due to some circumstances he is unable to make CGHS card after retirement. he had retired from Assam 2005. He is suffering for kidney problem since last 6years but now from 2months back every alternate days he have to take dialysis which is very expensive for my family because me and my mother is completely depend on him. so it is requested you what is the procedure to make the new CGHS card awaiting for your valuable reply because

I am a Central Government Employee and I have included my husband as my dependent. He is employed in a private limited co., where insurance card was issued in his name for medical treatment. Can I still include my husband's name in the dependent list for medical treatment. If yes, give me the rule copy or number

Sir, One of our employees had got admitted in P D Hinduja Hospital Mumbai for B/L profound HL and surgery was conducted on him. NUCLEUS [PK01]CI 24M/DOUBLE AR costing rs. 5,48,000/- was purchased for the surgical proceedure/implantation. Now the PAO of our department objecting the MR Bill asking this Office to intimate the CGHS rate with code No of above item may be intimated. Request kindly provide CGHS rate and Code No of above item at the earliest please.
