I am an propritor of M/s Vinayak Enterprises and i have filled the application form with all the document required for code No in Marol Office in the month of April 2011. They have never send any information. I have also given pay order with 20 Labour form.so i want say that why it takes so many month.

I have not received my ESIC smart card till yet.all formality has done as on 12-Jun-11 in Nandnagri[Delhi] center but i have not received my ESIC smart card till yet(18-Nov-11).i have complained as on 14-Jul-11 by complain no-M4N9A2 after that i have not find any resolution or my SIC smart card...Please do the needful for the same

I have not received my smart card till yet. I have done all formalities at ESI Hospital Satpur Nashik on 23/05/2010.My Co. Code is 33000347460011001 and my IP No. is 3301311222.

MISBEHAVED BY MR PRAVAS SAHU [pharmacist] ESIC (Rajgangpur) -
MR Pravas sahu (Farmasist) of ESIC in I.T.Colony, Rajgangpur, Sundargarh, Odisha. MR Pravas sahu doesn't know that how to talk with female patients and old age patients. He uses abusing language with them. Do you think, Employees are paying for that to abusing his/ her old father and mother from ESIC employees.
He is join ESIC due to his brother’s death. His brother was working here. Means it is only for government sympathy. I appreciate government decision. But he is taking this wrong way. So please look into this matter.
HE HAS NO SENSE HOW TO TALK WITH PATIENTS and OLD AGE PEOPLES.
PLEASE TAKE NECESSARY ACTION AGAINST HIM.
