Its been a week since my dads removal of his appendix and today he said he drank water and it was leaking out of his incision. What does that indicate ?

This can indicate a number of things. Most likely he just has fluid in his peritoneum [abdominal cavity] that is forced out of the incision when the stomach expands. It could also be that the appendix was not ditched up properly but that is unlikely and would be accompanied with pain. I suggest you see your doctor/surgeon as soon as possible.
