Although there might be some companies which might deliver a lesser percentage of accuracy but we also do have evidence of companies who deliver accuracy levels of 99%. All humans tend to make mistakes and the same holds true for Transcriptionists worldwide althogh they might understand the ascent and terminology well. But it is all experience that counts.
