The desire to smoke regardless of the negative consequences seems to be more like the 'self mutilation' tendencies of the "cutter" who achieves a secondary gain through the creation of physical pain that overrides other unwanted psychic emotional contents. Perhaps this risk taking in the face of potential harm has similar gains?
