Question about blood pressure reading

How do I talk to one of the doctors on the board of consultants? I am basically going for the first time and do not know.
I'm 27 years, 5'4tall and weigh about 120 lbs. I smoke about 3 cigerettes a day and I am working on quitting. My blood pressure has been wierd and I don't know if I need to talk about it to my doctor ...
