Hi Mr Rajesh, Choledochal cyst is an uncommon congenital anomaly that requires surgical treatment. Type IVa choledochal cyst is dilation of the extrahepatic and intrahepatic biliary tree.
Severe hepatobiliary complications may result from long-standing biliary cysts if left untreated. The incidence of biliary carcinoma in the choledochal cyst ranges from 2.5% to 26%. The goals of management are to relieve symptoms and prevent long-term complications of biliary cysts such as infections, pressure upon the portal vein, liver failure, and potential cancer in the bile ducts. Resection of the extrahepatic cyst is recommended for type IV cysts. If the intrahepatic cysts are confined to one side of the liver a concomitant liver resection may also be required.
Management involves complete assessment of the disease, surgical resection and surveillance for development of carcinoma.
