You have not mentioned the reason for the obstruction, therapy depends on the cause of obstruction and the type of obstruction. The most common cause of obstruction at the pelviuretric junction is a kidney stone. If the stone is causing obstruction with hydronephrosis than it needs to be removed. If it is small say about 3mm in diameter than lithotripsy is done where the stone is broken down into small fragments using external wave forms. If it is large than it has to be removed surgically
