I think what matters is the quality and purpose of surgery. Highlighting the issue of cosmetic surgery, I feel it is not the correct way. I have seen many surgeons mess up straight forward cases in an attempt to provide so called cosmetically appealing outcome.

SILS (single incision laproscopic surgery)appendectomy is being performed in reasonable numbers these days. It has all the advantages of laproscopic The only difficulty is the discomfort due to lack of triangulation. However the use of some new equipment has made SILS surgery relatively safe. I am sure we should keep up with the progress in surgery

Our group at Dr U Mohan Rau Memorial Hospital in Chennai have performed this surgery as early as 2003 and published in JIMA. Ref: JIMA,2004,Jul;102(7):360,362,364. Now in 2010, many reports of Lap surgery through single incision are being reported and this is increasingly accepted as a definite step forward.

Now in 2011-12 more and more Lap Surgeries are performed using a single incision through umbilicus- called SILS (Single Incision Lap. Surgery). But this needs lots of complicated and expensive instrumentation. But we did the "Scarless Appendicectomy" ijn 2003 using conventional lap instruments.
And now surgeons are also doing Lap Surgery through natural orifices , mainly through mouth and trans gastric - called NOTES.
