rales on auscultation. He has a history of IHD. CXR shows cardiomegaty
and pulmonary oedema.

inhaler for his asthma. ECG shows ST segment depression.

pain is no longer alleviated by rest or with GTN. Coronary angiogram
shows left main stem obstruction.
